Appointments

We are thrilled to have you consider our Pediatric Clinic Davenport for you child’s health needs. Below is some patient info for you to review on our Pediatric Clinic Davenport’s operations.

Late Arrival

We understand schedules can be disrupted. If you are more than ten minutes late for your appointment, we will attempt to accommodate you but reserve the right to ask you to reschedule so that the doctor can remain on time for the remainder of her patients.

Cancelled Appointments

We understand that occasional circumstances prevent you from keeping your scheduled appointment. We kindly ask that you give us at least 24 hours notice when canceling an appointment. Our scheduling coordinators will do their best to re-schedule you promptly.

After Hours Calls

If your child having a medical issue which cannot wait until our usual business hours, a doctor is always on call to help. Please be aware of that our doctor is home with her family after closing, and that after calls should be reserved only for the matters of an urgent nature which cannot safely wait until the next business day. If you need to call the doctor after hours, please call the office at (863) 232-5527 follow the instructions to page the doctor on call

Referrals

We kindly ask that you give us one week notice if your child will be seeing a specialist that requires at referral. We are happy to fax the referral to your specialist. When calling to request a referral, please give us the name of the doctor the type of specialist, the date and time of the appointment, the address, phone and fax number of the doctor and the reason for your upcoming appointment. If your appointment is of an urgent nature and you did not have an opportunity to give us notice (i.e. A broken bone), we will send your referral as soon as possible accommodate you.

Medication Refills

Please give the doctor at least three (3) business days to refill you prescription. If it is of an urgent nature, please leave a message starting with the receptionist.

Insurance and Billing

We are more than happy to submit all insurance forms for you and help you recover the most from your benefits. We are in the AvMed Health Plans, Blue Cross And Blue Shield of Florida/ Health Options, Inc., Comprehensive Health Management-Wellcare Health Plans, Inc., Global TPA, LLC, Humana/Choice Care, Integral Quality Care, Simply Healthcare Plans, Sunshine State Health Plans, United Health Care, Wellmed Medical Management/Physicians Health. and to any healthcare organization that in the future represents to CAQH either that Iam participating provider or that Iam in the process of being credentialed as a participating provider.

Office Policies

Please make sure that if your insurance requires you to declare a primary medical doctor that you call them and make change prior coming to your appointment them and make the change prior to coming for your appointment. Failure to do so may cause the insurance company to refuse payment causing you to become responsible for the medical bill. All outstanding balances and co-payments are due at the time of service. Co-pays and outstanding balances are collected at the time of the service. We accept cash, Checks, Master Card & VISA
